Applications may be submitted at any time. All applications must be approved PRIOR TO submitting reimbursement requests. Written notification will be sent indicating approval and the date for which you may begin applying for reimbursements.
The application package includes: application form, employee compliance letter and copy of provider’s State of Ohio license. Please enter the hours on the Parent and Child Schedules as 8:00-5:00 or 7:30-5:30.
When participants use primary and alternate child care providers (Example: School days, Non-School days), both providers should be listed on the application.
If any changes occur in providers or work schedules, a new application package must be completed and submitted as described above.
